COSMETIC SERVICES
1. What is a cosmetic service or procedure versus a medical procedure?
Traditionally, cosmetic treatments for cosmetic problems were defined as "treatments that provided enhancement of features to improve appearance with no significant benefit to structure and function of the skin, hair and nails." Most of the services listed here could be considered cosmetic, but a number also could have medical or corrective implications as well. For most of my career, which began in 1978, the difference between cosmetic and medical problems has been quite distinct with the demarcations or separations clearly defined. However, recently the term "cosmetic" has taken on unclear interpretations, primarily due to insurance companies setting a business policy that has placed traditional medical problems in a cosmetic category.
For example, whereas the evaluation and medical management of acne is still considered by most insurance carriers as a medical problem and is usually covered, the correction of the sequelae of acne as scars with laser resurfacing is considered cosmetic by most insurance carriers and is not covered.
Another example is where the treatment of facial spasms with Botox is usually considered a medical treatment; the use of Botox for frown lines of the glabella, forehead and crow's feet is a purely cosmetic concern. An even more relevant example in this era of photodamage is that of pigmented lesions. Based on a dermatological evaluation, a pigmented lesion may be an atypical nevus or a bad looking mole requiring surgical excision and is a medical condition whereas treatment of age spots and brown spots on the back of the hands and arms is considered a cosmetic treatment. A final example is the treatment of moles that are irritated by their size or location on the body which can be removed medically.
If the consultation has a reasonable medical basis, you will be informed by me at the initiation of the consultation once I am advised of the dermatologic concerns. Then, if you desire, your evaluations will be submitted to your insurance carrier as the medical problem that it is or you can elect to proceed with a non-insurance submitted cosmetic consultation. Once this determination is made, because of medical and legal obligations, it is not possible to switch between the two definitions once the consultation has been completed.
2. Is a cosmetic procedure handled differently in your practice than a medical procedure?
Our dermatology practice deals with a spectrum of medical and cosmetic problems. However, our goal is to provide immediate opportunities for evaluation and assistance in cosmetic areas. When you call in for a cosmetic consultation and indicate an interest in a certain area, you will be provided a packet of information regarding Beauty thru Health Dermatology, Medical Director and Board Certified dermatologist Clarence Wiley, M.D. and other pertinent information. If you desire, then you can schedule a complimentary consultation with our cosmetic staff. This one hour complimentary consultation will include an overall review of your cosmetic concerns, review of relevant videos with all questions answered and approximation of cost for these procedures. Finally, we will provide assistance with completing information for patient financing if you indicate a desire to proceed with any treatment.
After the complimentary consultation, you can schedule a second consultation with Dr. Wiley for further clarification of questions and/or treatment. The consultation with Dr. Wiley will be focusing on one basic cosmetic concern of 15 minutes of time at the cost of $75.00 or a comprehensive consultation of ½ hour for $150.00 where you can address one major concern as laser facial resurfacing or several minor cosmetic concerns. The cosmetic consultation fee will be applied to the procedure cost if scheduled within 30 days after the consult date for a procedure to be performed no more than 60 days from the date of the consultation. In essence, your consultation becomes complimentary by the performance of the procedure within 60 days of the consultation.
3. Do you have patient financing for cosmetic or even medical procedures?
To help you cover the reasonable cost for your procedure, we provide patient financing which can be submitted online in the convenience of your home or during your consult at the office.

BOTOX® Cosmetic
BOTOX® improves the appearance of:

BOTOX® is a medication used to temporarily paralyze muscle function. The name stands for Botulinum Toxin. It was created originally to treat certain eye conditions, mainly crossed eyes. BOTOX® was found to also relieve facial spasms and neck banding when injected. This resulted in smoother and younger looking skin around the injected areas.
Dr. Wiley has been utilizing BOTOX® since 1997 to erase wrinkles and relax muscles in specific facial areas, such as vertical and forehead frown creases and crow's-feet around the eyes. It will give a younger looking skin to the face without surgery.
BOTOX® is injected directly into the frown muscle or the wrinkle area. Within 5-10 days the muscles will relax and the wrinkles and frown will disappear or soften. The frustrated, tired and angry look on the face will go away and the patient will look younger with smoother skin.
It takes a few minutes to inject BOTOX® and its effects last for 2-5 months. The treatment has to be repeated as the wrinkles or frown will re-appear. With repeated injection the effects may last longer.

Sweating Clinic (medical BOTOX®)
BOTOX® has increasing number of medical uses. Botox has been used to treat excessive sweating of the palms, soles, and underarm areas (hyperhidrosis). Botox's benefits in helping to alleviate migraine headaches are well documented. Also, BOTOX® has provided benefit to patients with facial spasms. At Beauty thru Health Dermatology, Dr. Wiley utilizes BOTOX® for these and other dermatologic medical uses for which it has proven to be a safe and effective option.
Three percent (3%) of the U.S. population has hyperhidrosis. 4 million have axillary hyperhidrosis (or 51% of individuals with hyperhidrosis). 1.3 million people have severe primary axillary hyperhidrosis (or 32% of individuals with axillary hyperhidrosis.
Several primary axillary hyperhidrosis is defined as grade 3 (barely tolerable and frequently interferes with my daily activities) or grade 4 (intolerable and always interferes with my daily activities).
Although hyperhidrosis isn't life threatening, it isn't harmless either. Hyperhidrosis can significantly damage quality of life with disturbing effects on social relationships, career choices and mental well being.
Recently the FDA has approved Botox for the medical treatment of primary axillary hyperhidrosis. Treatment has been shown to provide decreased to essentially normal sweating and control for up to 4 to even 6 months. Results are equally impressive for hyperhidrosis of the palms and the soles.

CHEMICAL PEELS
Chemical Peeling is a technique used to improve the appearance of the skin. In this treatment, a chemical is applied to the skin that causes it to separate and eventually peel off. The newer, regenerated skin is usually smoother and less wrinkled than the old skin.
Dermasurgeons developed and have used various peeling agents for the last 100 years and are experts in performing multiple types of chemical peels.
Beauty thru Health Dermatology Medical Director and Board Certified Dermatologist, Dr. Clarence Wiley, has performed chemical peels for over 20 years. He was the first doctor in the State of Kansas to train in the BioMedic Program and perform Micro peels in 1992. OBAGI BLUE PEEL
The Obagi Blue Peel is an in-office procedure designed to significantly improve the appearance and health of your skin. Using a low concentration of the chemical (trichloroacetic acid) mixed with a special blue base to slow penetration, the Blue Peel allows Dr. Wiley or his trained staff to monitor the appropriate depth to effectively remove the thin surface layers of the aged, photodamaged, discolored blotchy skin from not only the face but also the neck, chest, hands, back, arms, and legs.
After the Blue Peel, you will have light swelling and your skin will begin to peel in 2 to 3 days. Healing may be completed 3 to 6 days for lighter exfoliative peels and 7 to 10 days for deeper peels. You may want to limit your normal activity during this time. COLLAGEN THERAPY
When time or injury takes away the collagen that supports your skin, the scarring, wrinkling, or aging that results can rob you of confidence, enjoyment, and self esteem.
Beauty thru Health Dermatology Board Certified Dermatologist, Dr. Clarence Wiley has used bovine (cow) source collagen (Zyderm and Zyplast) since 1981 for these corrections. Although the risks of allergy to Zyderm are low, an allergy test is required with a wait of 30 days before treatment.

KELOIDS
Keloids are scars that heal above the skin. When the skin is injured in the dermis, it may heal to produce too much connective tissue to repair or connect the damaged skin. Keloids form when there is too much production of repair tissue. Although anyone can develop a keloid from a skin injury, African-Americans are more predisposed to develop keloids. Some families have a history of keloids developing even with just minor trauma, as an insect bite or ingrown hair.
Common sites for keloids in African-Americans are on the earlobes from ear piercing, cheek area from acne scars, back of the head in African-American men, and upper chest and shoulders of African-American women - although anyone can get a keloid on any part of the skin. Treatment of a keloid depends on factors such as location, size, shape, and prior treatment. Generally, people predisposed to keloids should avoid elective surgeries, especially scalpel surgeries. Dermatological treatment may range from intralesional cortisone therapy to silicone gel patches to topical imiquimod, to earlobe reshaping reconstructive surgery to CO2 and vascular laser surgery. Careful evaluation of a particular keloid by Dr. Wiley allows him to provide options of treatment that are available to you for maximum medical and cosmetic results.

LASER/LIGHT THERAPY AND SURGERY
From his early training in 1978 as a resident in dermatology at Barnes Hospital, Washington University Medical Center in St. Louis, MO, Dr. Clarence Wiley developed a keen interest in light therapy. There, under the guidance of Dr. Arthur Z. Eisen, he was first exposed to light therapy for light responsive problems such as eczema, psoriasis, and vitiligo.
It was his interest in vitiligo therapy that stimulated him to continue to study and use light therapy and in 1982 develop the first Vitiligo Therapy Center in the mid-west in Oklahoma City.
Experiencing the benefits of light therapy for these medical problems for his patients, Dr. Wiley's interest was especially driven by the first introduction of lasers. Dr. Wiley traveled to Cincinnati, Ohio to train with Dr. Leon Goldman, "The Father of Skin Laser Surgery" in 1982. This and subsequent training with the "Master" laid the foundation for his career as one of the first skin laser surgeons in the country.
From his initial training in laser surgery, Dr. Clarence Wiley has pioneered and developed laser skills and techniques. He was one of the first laser surgeons in Oklahoma when he started performing laser surgery in 1982. In June 1986 when he relocated to Kansas, Dr. Wiley was one for the first laser surgeons in the state of Kansas. SKIN RESURFACING

Ultraviolet light exposure, smiling and squinting and aging all contribute to wrinkles that appear on your face making you look tired, and/or older. Wrinkles used to be a fact of life. Dr. Clarence Wiley uses resurfacing methods from peels to advanced Erbium-YAG and CO2 lasers to perform cosmetic skin resurfacing to reduce wrinkles. WHAT SHOULD I EXPECT FOLLOWING THE TREATMENT?
Once the treatment is complete, the area exposed to the laser is cleaned and you are free to return to your normal routine. Some patients may experience a slight reddening around hair follicles following the treatment, which typically disappears within minutes to a few hours. It is always recommended to avoid sun exposure and to use sunscreen following the laser treatment. Hair follicles affected by the laser are lifeless and your body will attempt to cast them off over a period of time. It will seem that the hair continues to grow for one to two weeks after the procedure, but in fact these lifeless hairs will easily fall out.
PSEUDO-FOLLICULITIS (Shaving bumps, Razor bumps, Ingrown hairs)
Pseudo-folliculitis (PF), commonly known as shaving bumps, is an inflammatory condition, widespread among individuals with thick curly hair. PF, therefore; occurs in African-American men and women with facial hair problems. As the hair grows out of the skin, it curls and re-enters the skin. The skin reacts to it as a foreign object and becomes inflamed and irritated, creating bumps and discoloration. Often hairs start curling even before growing out of the skin, creating in-grown hairs. These bumps become quite large, causing discomfort, discoloration and scars to the patient.
Sometimes relief can be achieved with the use of topical steroids and exfoliating agents. Growing the hair above the skin is sometimes curative as well. More commonly though, the ingrown hair is pulled out after each shaving session. Although this is a solution, it can be painful and time consuming and result in discoloration and scarring of the skin.
If you are in an occupation such as the military where it is a "requirement" to shave and have a PF problem, then laser hair removal should seriously be considered as an option!

LASER VEIN THERAPY

HOW DOES THE GENTLE YAG WORK?
The Gentle YAG laser produces 1064 nm wavelength. While the rest of the skin is almost completely transparent for this wavelength, it is highly absorbed in blood. Thus, the laser beam passes through the skin and penetrates the targeted vessel. The laser energy heats up and coagulates blood inside the vessel. Unsupported, vessel walls collapse and seal. The vessels later dissolve within the body.

WHAT CAN I EXPECT DURING THE TREATMENT?
You will experience a stinging, burning sensation during the treatment which resolves immediately after the treatment is over. In most cases, we recommend the topical anesthetic cream which may completely eliminate or reduce any discomfort.
WHAT SHOULD I EXPECT AFTER THE PROCEDURE?
The appearance of the treated vessels will dramatically change turning from blue and purple into light red. Some of the vessels may bulge. However, any inflammation will usually disappear within a couple of hours. The redness of the treated vessels will gradually disappear as the healing process occurs, which may take anywhere between 2 and 6 weeks depending on the severity of the veins.
The post-operative treatment regime will include wearing support hose, avoiding medication containing aspirin or other blood thinners and wearing sunscreen. Make sure to discuss these options with your laser therapist prior to the treatment.
HOW MANY TREATMENTS WILL I NEED?
Depending on the severity of the veins, you may require between one and three treatments six weeks apart IN AN AREA. The treatment is performed on the existing vessels, diminishing or completely eliminating their appearance. However, the development of new veins cannot be prevented. If your body tends to develop vascular problems you will most likely come back for additional treatments.

SCLEROTHERAPY (Spider veins, Telanglectasia)
Spider veins are unwanted blood vessels on the legs and are generally an affliction of active women, although men may also be affected.
That means that 60% of women more than 25 years of age may have unwanted leg veins. Predisposing factors to a future vein problem are heredity, pregnancy, birth control pills, jobs where you are on your feet all day and injury to the legs.
Unwanted blood vessels in the face may be due to rosacea or photodamage but veins can develop at other areas on the body as well.
Sclerotherapy or the removal of unwanted veins by injection has long been an effective method of removal by dermasurgeons. Dr. Clarence Wiley has been performing sclerotherapy for over 24 years and is one of the premier phlebologists in the state.
The small veins known as spider veins can be carefully injected with a solution that causes these veins to close. Then these veins are reabsorbed or removed from the body with the appearance of a bruise going away. The disappearance of the vein may leave pigment from healing that can take several months to go away.

OTHER LASER THERAPY
AGE SPOTS
Just about everyone has a few obvious brown spots on their skin. Freckles, age spots, liver spots and brown birthmarks may be quite conspicuous and can detract from your skin's natural beauty.
Today, age spots and other pigmented lesions can be successfully lightened or removed with modern laser/light technology. Dr. Clarence Wiley has over 22 years laser experience and the MedLite IV and Aura lasers to help remove the brown pigment with the energy of light.
Although the majority of all common pigmented lesions are removed in one treatment, some may require several treatments. The location, size, type of pigmented lesion and your expectations are all fully considered in discussing the best options for removal of your age and brown spots.
PIGMENTED LESIONS
At Beauty thru Health Dermatology, we have two (2) of the most advanced lasers (MedLite IV, Aura) available today for the removal of pigmented lesions.
Melanin is what gives our skin its color. Dark or light complected, we all have varying amounts of melanin. Pigmented lesions are dark in color simply because melanin is abnormally concentrated in one area of the skin. High concentrations of melanin can be due to various factors. Some types are present at birth, but most occur with age or as a result of overexposure to the sun's damaging rays.
Pigmented lesions can be successfully lightened or removed with modern laser technology. A laser is designed to produce one or more specific light wavelengths which are absorbed by the pigment. Pigmented lesions are either lightened or removed when the laser light passes through the skin but is absorbed by abnormal concentrations of melanin. MOLES
40-60% of African-American men and women have dark growths on the face which are usually called "moles", but represent a skin condition called seborrheic keratosis (SK).
SKs are a hereditary skin condition and appear as dark brown to black shiny to rough bumps that start as small pinpoint growths or a flat brown blotch. Each SK can get larger with time or may stop at apple seed size or the size of a half-dollar. SKs most commonly occur on the face, neck and trunk.
SKs can be itchy and very easily irritated by you or your clothes rubbing. An irritated SK can resemble and must be distinguished form malignant melanoma, which does occur in African Americans. Melanoma skin cancer is usually fatal if not diagnosed and completely excised at the earliest time. We will carefully examine the growths.
In Dr. Wiley's over 26 years experience, the best form of treatment for SKs is very gentle freezing of the bumps using liquid nitrogen (cryosurgery) with minimal discomfort. In his experienced hands, these growths come off after freezing usually in 5 to 12 days, leaving a temporary light to dark spot that returns to normal color over several months.
However, some flat-type seborrheic keratosis or ephelides may respond better to pigmented laser therapy (Aura or MedLite IV). A small test with both cryosurgery and laser may be warranted in some cases.
"True moles" or nevi are a collection of pigment cells. They can closely resemble SKs to you or non-dermatologic physicians. Light or white areas that develop in your normal skin color can be one or many disorders. However, one of the most significant and devastating causes of loss of skin color is vitiligo.
Vitiligo is a disorder where the skin color can turn light to completely white in the skin, but also in the hair. This is due to destruction of the pigment cells (melanocytes) in these areas which make a brownish-black skin pigment (melanin).
